Compounded Medication Price Index 2026
What compounding pharmacies actually charge, drug by drug and form by form, from the price lists they share with us for quoting. Aggregated across pharmacies that can take a quote request today; no pharmacy is named. Free to cite with a link.
The headline: the same drug costs 2.7 times more at some pharmacies than others
Compounded medications have no list price. Each pharmacy sets its own, and because patients rarely compare, the gap persists. Across the 33 drug-and-form pairs with enough listings to measure, the typical high-end pharmacy charges a median of 2.7× what the low-end pharmacy charges per capsule, gram, troche or vial, for the same drug in the same form.
The widest gaps are not exotic drugs. They are everyday compounds where one pharmacy prices a 90-count fill the way another prices a 30-count.

Method
- Source. Price lists shared with Compounding Finder by licensed compounding pharmacies for quoting patients. Only pharmacies that can currently receive a quote request are counted; pharmacies that have left the network are excluded even though their lists remain on file.
- Cleaning. Listings priced under $10 (a per-unit price typed into a total) or at $5,000 and above (placeholder "call us" values) are dropped, and identical listings from the same pharmacy are counted once.
- Forms. Each listing is assigned to one dosage form from its formulation and product name. A drug-and-form pair is published only with at least 3 pharmacies and 5 listings.
- Per-unit prices. Derived from the listing's quantity and total, or from an explicit per-unit price where a pharmacy states one and it differs from the total. Values more than eight times the median are treated as package prices carrying a unit label and excluded. With ten or more values the published range is the 10th to 90th percentile. Injections, nasal sprays and eye drops are listed per vial or bottle in many sizes, so they show fill prices and no per-unit figure or spread.
- Single ingredients only. A medication's figures count only listings for that medication on its own. Combination products, such as a troche that also contains oxytocin, are left out, and a combination medication counts only listings that contain all of its ingredients.
- Not included. Shipping, dispensing fees and prescriber visits. GLP-1 medications are excluded because their availability is governed by FDA shortage rules rather than price.
Questions about the index
Where do these prices come from?
From the price lists compounding pharmacies share with Compounding Finder so that we can quote patients. We count only pharmacies that can currently take a quote request, drop placeholder and per-unit-typed-as-total rows, and require at least 3 pharmacies and 5 listings before a drug-and-form pair appears. Figures are as of October 2026.
What does "typical per-unit price" mean?
The 10th to 90th percentile of the per-unit price across every listing for that drug and form, once there are ten or more listings, so a single outlier cannot stretch the range. For smaller sets it is the full range. The median is the middle listing.
What is the spread ratio?
The top of the typical per-unit range divided by the bottom. A spread of 3.0 means the higher-priced pharmacies charge about three times what the lower-priced ones charge for the same drug in the same form. It is the clearest measure of how much comparing pharmacies is worth.
Why are pharmacies not named?
Pharmacies share their price lists with us for quoting, not for publication, so the index reports the market without identifying any pharmacy. A quote request returns named prices for your own prescription.
Can I use these figures?
Yes. The index is free to cite and republish with a link to this page. The full table is available as a CSV at /compounded-medication-price-index.csv equivalent below, and is regenerated whenever the underlying price lists change.
