Both Wegovy and compounded semaglutide contain the active ingredient semaglutide, a GLP-1 receptor agonist. However, they differ in important ways regarding FDA approval, regulation, and who can access them.

FDA Approval
Wegovy: FDA-approved for chronic weight management in adults with obesity (BMI 30+) or overweight (BMI 27+) with at least one weight-related condition.
Compounded semaglutide: Not FDA-approved. Prepared by licensed compounding pharmacies under federal and state regulations.
Manufacturer
Wegovy: Manufactured by Novo Nordisk under strict FDA manufacturing standards (cGMP).
Compounded semaglutide: Prepared by individual 503A or 503B compounding pharmacies, regulated by state boards and/or the FDA.
Available Forms
Wegovy: Pre-filled injectable pen only, in fixed dose strengths (0.25mg, 0.5mg, 1mg, 1.7mg, 2.4mg).
Compounded semaglutide: Injectable, sublingual, troche, and oral capsule forms. Custom dose strengths available.
Insurance Coverage
Wegovy: May be covered by insurance, though coverage varies widely. Manufacturer savings programs may be available.
Compounded semaglutide: Typically not covered by insurance.
Who Can Access Compounded Semaglutide?
Since the FDA shortage resolution in February 2025, compounded semaglutide is only available to patients with a documented clinical need:
- Allergy to inactive ingredients in Wegovy
- Need for a non-standard dose or dosage form
- Other documented medical necessity
Cost or personal preference alone does not qualify a patient for compounded semaglutide.
Quality Considerations
Wegovy undergoes rigorous FDA quality control. Compounded semaglutide quality depends on the individual pharmacy. Look for pharmacies with PCAB accreditation and third-party testing.
The FDA has warned that some compounded products use semaglutide sodium salt, which is a different form than the FDA-approved semaglutide base. Verify your pharmacy uses the correct form.
