- April 2025: The enforcement discretion period ended for 503A compounding pharmacies preparing semaglutide.
- 2026: Compounding pharmacies may still prepare semaglutide for patients with legitimate clinical needs.
What Changed After the Shortage Ended
During the shortage, compounding pharmacies had broader latitude to prepare semaglutide under Section 503A of the Federal Food, Drug, and Cosmetic Act. Now that the shortage is over, compounding is permitted only when there is a documented clinical need that cannot be met by the commercially available product.
Who Can Still Get Compounded Semaglutide?
A licensed prescriber must document one of these clinical justifications:
- Allergy or sensitivity to inactive ingredients in branded Ozempic, Wegovy, or Rybelsus
- Need for a non-standard dosage strength not commercially available
- Requirement for an alternative dosage form (sublingual, troche, oral capsule)
- Other documented medical necessity
Important: Cost or preference alone does not qualify a patient for compounded semaglutide.
503A vs. 503B Pharmacies
503A pharmacies compound medications based on individual patient-specific prescriptions. They are regulated primarily by state pharmacy boards.
503B outsourcing facilities can compound larger batches without patient-specific prescriptions and are registered with and inspected by the FDA.
Both types are legal pathways for compounded semaglutide, with different regulatory oversight.
