Why trimix is only available compounded

Trimix combines three medicines: alprostadil (prostaglandin E1), papaverine, and phentolamine. Single-ingredient alprostadil is sold as FDA-approved products, but the three-drug combination is not. It is prepared only by licensed 503A compounding pharmacies from a valid prescription.

That is also why the concentration is not standardised. The prescriber sets the ratio and the strength to match the dose a patient has been titrated to, which means two trimix prescriptions can look nothing alike on paper.

What compounded trimix costs

That is a wide band, and the spread is real rather than noise. It is driven by:

• Vial size. A larger multi-dose vial costs more up front and usually less per dose.
• Concentration and ratio. A higher-concentration mix uses more active ingredient. Bimix (two drugs) and quad-mix (four) are priced differently again.
• Sterile compounding requirements. Trimix is an injectable, so it must be prepared in a sterile facility. That is a different cost base from a capsule or a cream, and it is why the field of pharmacies is smaller.
• Cold-chain shipping. Trimix is typically refrigerated. Some pharmacies include cold-pack shipping; others add it.

Compare on cost per dose, not per vial. A larger vial at a higher price is frequently the cheaper prescription once you divide by the number of doses it holds — but only if you will use it within its beyond-use date.

Ask about these before you compare

Two vials at the same stated price are not necessarily the same purchase:

Beyond-use date. A compounded sterile preparation has a defined shelf life. A cheaper large vial is not cheaper if half of it expires unused.

What is included. Some pharmacies supply syringes and alcohol swabs with the vial; others do not. On a small order this is a real difference.

Storage and shipping. Ask how it ships, whether a signature is required, and what happens if a cold-pack delivery is delayed.

Prescriber supervision matters here

Trimix is dosed individually. Prescribers typically start low and titrate, and many require an in-office test dose and injection training before a patient self-injects.

The safety point worth stating plainly: an erection lasting more than four hours (priapism) is a medical emergency and needs prompt care. Discuss dosing, technique, and what to do about a prolonged erection with your prescriber before you start.
