Mast Cell Stabilizer Drug Class

Mast cell stabilizers prevent mast cells from releasing histamine and other inflammatory mediators. In compounded form, they are critical for patients with Mast Cell Activation Syndrome (MCAS) and mastocytosis who need customized dosing or delivery forms.

How It Works

Mast cell stabilizers like cromolyn sodium and ketotifen inhibit degranulation of mast cells, preventing the release of histamine, leukotrienes, prostaglandins, and tryptase that trigger allergic and inflammatory cascades.

Why Compounded?

Cromolyn sodium oral solution is commercially available but expensive and limited in concentration. Ketotifen is only available in the US as eye drops. Compounding enables oral ketotifen capsules, liquid suspensions, nasal sprays, and topical formulations at therapeutic doses.

Common Uses

  • Mast Cell Activation Syndrome (MCAS)
  • Systemic mastocytosis
  • Allergic conditions and urticaria
  • Eosinophilic esophagitis
  • Irritable bowel syndrome (IBS)
  • Interstitial cystitis
  • Food allergies and intolerances

Common Compounded Medications

  • Ketotifen Oral Capsules (1–2mg)
  • Ketotifen Liquid Suspension
  • Cromolyn Sodium Oral Solution (customized strength)
  • Ketotifen Nasal Spray

Regulatory & Safety Notes

Ketotifen is not FDA-approved in the US for systemic use (only as an ophthalmic solution). Oral ketotifen is commonly compounded for MCAS treatment. Cromolyn sodium oral solution (Gastrocrom) is FDA-approved but expensive; compounded versions are cost-effective alternatives. Prescriptions are required.
