Adrenal Hormone / Androgenic Precursor Drug Class

Adrenal hormones including DHEA (dehydroepiandrosterone) and cortisol are produced by the adrenal cortex. DHEA is a precursor to testosterone and estrogen, while cortisol regulates the stress response and immune function. Compounded adrenal hormones are used in adrenal insufficiency, BHRT, and autoimmune conditions.

How It Works

DHEA is peripherally converted to testosterone and estrogen in tissues. It also acts directly on GABA receptors as a neurosteroid. Cortisol (hydrocortisone) binds to glucocorticoid receptors, regulating inflammation, glucose metabolism, and the stress response.

Why Compounded?

Therapeutic-dose DHEA beyond OTC supplements, low-dose physiologic cortisol (hydrocortisone), DHEA vaginal suppositories, and customized adrenal support preparations all require compounding.

Common Uses

  • Adrenal insufficiency and Addison's disease
  • DHEA replacement in menopause
  • Systemic lupus erythematosus (SLE)
  • Low libido and sexual dysfunction
  • Cognitive and mood support
  • Adrenal fatigue protocols

Common Compounded Medications

  • DHEA Capsules (custom dose)
  • DHEA Transdermal Cream
  • DHEA Vaginal Suppositories (Intrarosa equivalent)
  • Hydrocortisone Physiologic Dose Capsules
  • Pregnenolone Capsules

Regulatory & Safety Notes

DHEA is classified as a dietary supplement at OTC doses but requires a prescription in compounded therapeutic formulations. Cortisol (hydrocortisone) compounded at physiologic doses is prescribed for adrenal insufficiency. Patients should undergo appropriate adrenal function testing before beginning therapy.
