Trimethoprim-sulfamethoxazole (TMP-SMX) is a broad-spectrum antibiotic combination widely used in equine medicine for respiratory, urinary, soft tissue, and wound infections. The dose required for a horse (typically 15–30mg/kg twice daily) translates to a very large number of tablets — often 20–40 tablets per dose — making conventional tablet administration impractical.
Compounded trimethoprim-sulfa oral powder allows the required dose to be measured and mixed into feed, making administration straightforward. The powder formulation also allows dose adjustments for large versus small horses, ponies, and foals.
Trimethoprim and sulfamethoxazole inhibit two sequential steps in bacterial folate synthesis. Sulfamethoxazole inhibits dihydropteroate synthase, while trimethoprim inhibits dihydrofolate reductase. Blocking both steps sequentially produces a synergistic bactericidal effect more potent than either drug alone.
Because mammalian cells obtain folate from diet rather than synthesizing it, the combination is selectively toxic to bacteria.
In horses, TMP-SMX powder is used for respiratory infections (including strangles, pneumonia), urinary tract infections, wound and soft tissue infections, Pneumocystis pneumonia in foals, and as prophylaxis in immunocompromised horses. TMP-SMX is generally well tolerated in horses. The most common concern is diarrhea from disruption of hindgut microbiome; administering with feed and monitoring manure consistency is advisable. Rare adverse effects include skin reactions and bone marrow suppression with prolonged use.
Some horses show reluctance to eat feed with TMP-SMX powder; palatability enhancers (molasses, apple sauce) can help.
