HOW IT WORKS

Mast cell stabilization

Prevents mast cells from degranulating — stopping the release of histamine, leukotrienes, and other inflammatory mediators before they trigger symptoms.

H1 receptor antagonism

Blocks histamine H1 receptors directly at target tissues — a second line of action that manages symptoms even when some mediator release occurs.

Oral suspension, capsules, chewable tablets

How it works

Pimobendan sensitizes cardiac myofilaments to calcium and inhibits phosphodiesterase III, increasing cardiac contractility and causing vasodilation.

Why compounded?

Very small and toy breed dogs require doses below those available in commercial tablets. Compounded oral suspension enables precise weight-based dosing.

Compounded Pimobendan Oral Suspension for Dogs

Pimobendan is the cornerstone of heart failure management in dogs and is standard of care for dilated cardiomyopathy (DCM) and mitral valve disease (MVD). The commercial product, Vetmedin, is available in 1.25mg, 2.5mg, 5mg, and 10mg capsules or chewable tablets.

Very small dogs — Chihuahuas, toy poodles, Maltese, and other breeds under 5kg — often require doses below 1.25mg that cannot be accurately achieved by splitting capsules. Compounded oral suspension in concentrations such as 0.5mg/mL allows these patients to receive precisely dosed pimobendan, which is critical for managing cardiac function and quality of life.

How it Works

Pimobendan has two distinct mechanisms: it is both a calcium sensitizer and a phosphodiesterase III (PDE-III) inhibitor. As a calcium sensitizer, it increases the sensitivity of cardiac myofilaments to calcium ions, improving myocardial contractility without increasing intracellular calcium (and thus without increasing oxygen demand). As a PDE-III inhibitor, it causes vasodilation, reducing both preload and afterload on the failing heart.

The combination of positive inotropy and vasodilation makes pimobendan uniquely effective for canine heart failure.

Common Uses

Compounded pimobendan suspension is used in dogs for congestive heart failure secondary to dilated cardiomyopathy (DCM) or mitral valve disease (MVD). It is also now used in preclinical (asymptomatic) MVD patients with certain echocardiographic criteria, following the EPIC trial findings, to delay onset of heart failure.

Side Effects

Pimobendan is generally very well tolerated. The most common side effects are poor appetite and mild lethargy, which are usually transient. GI signs including vomiting and diarrhea can occur.

At very high doses, pimobendan may cause tachycardia. It should be used cautiously in dogs with hypertrophic cardiomyopathy (HCM), where increasing contractility could worsen outflow obstruction.
