Pimobendan is the cornerstone of heart failure management in dogs and is standard of care for dilated cardiomyopathy (DCM) and mitral valve disease (MVD). The commercial product, Vetmedin, is available in 1.25mg, 2.5mg, 5mg, and 10mg capsules or chewable tablets.
Very small dogs — Chihuahuas, toy poodles, Maltese, and other breeds under 5kg — often require doses below 1.25mg that cannot be accurately achieved by splitting capsules. Pimobendan has two distinct mechanisms: it is both a calcium sensitizer and a phosphodiesterase III (PDE-III) inhibitor. As a PDE-III inhibitor, it causes vasodilation, reducing both preload and afterload on the failing heart.

Compounded pimobendan suspension is used in dogs for congestive heart failure secondary to dilated cardiomyopathy (DCM) or mitral valve disease (MVD). Pimobendan is generally very well tolerated. The most common side effects are poor appetite and mild lethargy, which are usually transient. GI signs including vomiting and diarrhea can occur.
At very high doses, pimobendan may cause tachycardia. It should be used cautiously in dogs with hypertrophic cardiomyopathy (HCM), where increasing contractility could worsen outflow obstruction.
