Compounded Enalapril Oral Liquid for Dogs & Cats

Enalapril is an ACE (angiotensin-converting enzyme) inhibitor used in dogs and cats for heart failure, hypertension, and chronic kidney disease (CKD). While enalapril tablets (Enacard, Vasotec) are available commercially, very small dogs and cats often require doses below what can be accurately achieved by tablet splitting.

Compounded enalapril oral liquid allows milligram-precise dosing for any size patient, with flavoring for improved palatability. It is particularly important in cats with CKD, where enalapril is often used for its renoprotective effects on glomerular filtration, and where cats notoriously resist tablet administration.

How it Works

Enalapril is a prodrug that is converted in the liver to enalaprilat, the active ACE inhibitor. Enalaprilat blocks the conversion of angiotensin I to angiotensin II, reducing systemic vasoconstriction, lowering blood pressure, decreasing aldosterone secretion, and reducing intraglomerular pressure in the kidneys.

The reduction in glomerular hypertension is particularly valuable in CKD patients, as it slows progression of proteinuric kidney disease.

Common Uses

Compounded enalapril liquid is used in dogs and cats for congestive heart failure (as an adjunct to other cardiac drugs), hypertension, chronic kidney disease (CKD) with proteinuria, and mitral valve disease. It is often combined with furosemide and pimobendan in dogs with heart failure.

Side Effects

The most common side effect is hypotension (low blood pressure), particularly at initiation or with concurrent diuretic use. Signs include weakness, lethargy, and fainting. Starting at a low dose and monitoring blood pressure reduces this risk.

ACE inhibitors can worsen kidney function in some patients, particularly those with renal artery stenosis or severe dehydration. Renal function and electrolytes should be monitored after starting therapy.
