Compounded Chlorambucil Capsules for Cats

Chlorambucil is an alkylating chemotherapy agent used in cats primarily for the treatment of small cell (low-grade) gastrointestinal lymphoma and inflammatory bowel disease when it has not responded to corticosteroids alone. It is typically used in combination with prednisolone as part of a "chlorambucil-pred" protocol.

The commercial formulation (Leukeran) comes only in 2mg tablets, which is often too high for the precise low-dose protocols cats require. Compounding pharmacies prepare chlorambucil in smaller doses (commonly 0.5mg–1mg capsules) that allow dose adjustments tailored to the individual patient's weight and response.

How it Works

Chlorambucil is a nitrogen mustard alkylating agent that cross-links DNA strands, preventing DNA replication and cell division. It preferentially affects rapidly dividing cells, including malignant lymphocytes, making it effective against lymphoid tumors.

At the low doses used for feline IBD and small cell lymphoma, it provides immunomodulation and anti-tumor activity with a manageable side effect profile.

Common Uses

Chlorambucil compounded capsules are used almost exclusively in cats for small cell (low-grade) lymphoma and refractory inflammatory bowel disease, typically in combination with prednisolone. This combination ("chlorambucil-pred") is the standard of care for feline small cell lymphoma and achieves remission in a high proportion of patients.

Side Effects

At the low doses used for feline IBD/lymphoma, chlorambucil is generally very well tolerated. The most common side effects are mild gastrointestinal signs (vomiting, decreased appetite) and, rarely, bone marrow suppression (low blood cell counts).

Routine CBC monitoring (every 1–3 months) is recommended to detect early bone marrow suppression. Neurologic signs (tremors, seizures) are rare but possible at higher doses.
