Budesonide is a locally-acting corticosteroid with high topical activity and extensive first-pass hepatic metabolism, resulting in significantly lower systemic bioavailability compared to prednisolone. Commercial budesonide capsules (Entocort) are formulated for human dosing (3mg), which is too large for cats. Compounding pharmacies prepare budesonide in smaller doses (commonly 0.5mg–1mg) in appropriately sized capsules that can be administered directly or opened and mixed with food.

Budesonide has fewer systemic side effects than prednisolone, but is not entirely free of them. Some cats on long-term budesonide therapy develop mild increases in blood glucose, adrenal suppression, or weight gain. These are less severe and less frequent than with equivalent prednisolone doses.
Annual monitoring of blood glucose and adrenal function (ACTH stimulation or low-dose dexamethasone suppression) is recommended for cats on long-term therapy.
