How it works

Clonidine is an alpha-2 adrenergic receptor agonist. When applied topically, it activates peripheral alpha-2 receptors in the skin, reducing the release of substance P and other pain mediators from nociceptors. This peripheral mechanism provides localized analgesia without the central sedation and blood pressure effects of oral clonidine. Clinical trials show it selectively benefits patients with intact (not completely destroyed) sensory nerve fibers.

Why compounded?

The only commercially available topical clonidine (Qutenza-adjacent; Catapres-TTS is a patch for systemic delivery) is a transdermal patch designed for systemic blood pressure control. Compounding pharmacies prepare topical clonidine in gels and creams at concentrations optimized for local pain relief with minimal systemic absorption, customized for specific body areas.

What is

Clonidine Topical

?

Clinical research including an NIH-funded trial by Dr. David Wiles demonstrated that topical clonidine 0.1% gel significantly reduced foot pain in diabetic neuropathy patients with preserved C-fiber function. It's now used at many pain clinics as part of multimodal topical pain protocols, often combined with ketamine, gabapentin, or other agents in custom transdermal formulations.
