Alpha lipoic acid is a universal antioxidant active in both water-soluble and fat-soluble environments. It regenerates other antioxidants (vitamins C, E, glutathione), chelates heavy metals, improves glucose uptake in peripheral tissues, and reduces oxidative stress in nerves. IV administration achieves plasma concentrations 10–20x higher than oral dosing, maximizing its neuroprotective and antioxidant effects.
Injectable ALA is not commercially available in the US (it is approved and widely used in Germany for diabetic neuropathy). Compounding pharmacies prepare sterile IV and IM formulations, typically 300–600mg/mL solutions or lyophilized powder for reconstitution, using pharmaceutical-grade ALA.

IV alpha lipoic acid is a cornerstone treatment at integrative medicine and IV nutrient therapy clinics. In Germany it has been an approved treatment for diabetic neuropathy for decades. US clinics use compounded ALA for neuropathy, liver support, heavy metal chelation support, and as part of high-dose antioxidant protocols. The R-lipoic acid form is considered the more bioactive enantiomer.
Diabetic peripheral neuropathy, antioxidant IV therapy, liver disease/hepatitis support, heavy metal detoxification support, metabolic syndrome, insulin resistance.
Neuropathy: 300–600mg IV over 30–60 minutes, 2–5 times weekly. Maintenance: 1–3 times weekly or monthly. IM injection: 150–300mg IM. Dose per prescribing physician's protocol.
Rapid IV infusion can cause burning sensation, tingling, or nausea—slow infusion minimizes this. Thiamine (B1) depletion possible with high doses; supplement thiamine. Hypoglycemia risk in diabetics. Generally well tolerated with slow infusion.
